1. ETIOLOGY AND PATHOGENESIS
대략 인구의 0.5에서 2.2%까지 발생한다.(in USA)
주요 etiologic factor : pressure, friction, shearing forces, and moisture.
Pressure or force per unit area는 가장 중요한 요소이고 눌린 시간도 중요하다. 지속적인 압력은 조직의 괴사를 초래한다.
The pathophysiology of pressure ulcer formation is summarized.
Related risk factor : prolonged immobilization, sensory deficit, impaired consciousness, circulatory disturbance, poor nutrition, and chronic diseases.
No specific laboratory findings. The diagnosis is made clinically.
2. CLINICAL FINDINGS
1) History : 위험요인들을 확인해아 한다.
2) Physical Examination
systemic disease 확인 : anemia, chronic cardiac or respiratory disease, and neurologic disorders.
3) Ulcer Evaluation
Staging is based on anatomic depth of soft-tissue damage.
4) Related Physical Findings
Tenderness, erythema, edema and warmth of surrounding skin, exudate, and foul odor는 감염을 시사한다. Fever and declining mental or physical status는 bacteremia or osteomyelitis를 의심할 수 있다. Spasticity secondary to inflammation and infection는 muscle contractures와 joint deformity를 일으킨다. Weakness와 anemia, dehydration소견은 profound loss of fluid and protein로 인한 이차적 결과이다.
