한국어 초록
목적 : 본 연구에서는 체계적 고찰을 통해 노인 운전자의 운전수행을 위한 다양한 중재의 효과를 분석하고 자 한다. 연구방법 : 2005년 1월부터 2015년 4월까지 10년간의 문헌을 MEDLINE, Google Scholar 데이터베이스 에서 검색하였다. 검색에 사용된 키워드는 다음과 같다; ‘Automobile driving’ (MeSH term), ‘Aged’ (MeSH term), ‘Occupational Therapy’ (MeSH term), ‘Treatment Outcome’ (MeSH term). 최종적 으로 포함기준에 적합한 7개의 연구를 선정하였다. 결과 : 포함된 연구의 근거수준은 가장 높은 수준인 수준I이 71.4%(5편), 수준Ⅱ는 28.6%(2편)를 차지했 다. 중재 방법은 다음과 같이 5가지 유형으로 분류하였다; (1) 인지 훈련, (2) 신체적 훈련, (3) 교육 프로그램, (4) 시뮬레이터 훈련, (5) 도로운전 훈련. 인지 훈련의 경우 추론 훈련과 처리속도 훈련에서 50.0%의 사고감소율을 보였고 신체적 훈련의 경우 운전수행이 향상되었으며 운전 실수 감소를 보였다. 교육 프로그램만 실시한 중재는 유의한 향상을 보이지는 않았으나 도로주행훈련과 결합하여 실시하였을 때 효과성이 증가하였다. 신체적 훈련, 운전 시뮬레이터 훈련 그리고 도로주행훈련에서는 운전수행이 향 상되었다. 결론 : 이번 연구를 통해 노인 운전재활 중재의 다양한 유형을 알 수 있었다. 운전수행 향상을 위해 작업치 료사는 노인 운전자에게 적합한 중재를 제공해 주는 것이 중요하다. 따라서 국내 임상 영역에서도 다양 한 운전재활 훈련이 개발되어야 할 것이다.
영어 초록
Objective : The aim of this study is to investigate the effectiveness of different interventions applied for driving performance of older drivers. Methods : Previous studies were found from an electronic database(e.g., MEDLINE, Google Scholar) published within a ten-year period from between January 2005 to April 2015. The following key terms were used: ‘automobile driving’ (MeSH term), ‘Aged’ (MeSH term), ‘Occupational Therapy’ (MeSH term) and ‘Treatment Outcome’(MeSH term). Seven articles met the inclusion criteria. Results : The articles reviewed included five Level I evidence articles(71.4%) and two Level Ⅱ articles(28.6%). Five intervention themes emerged from the research: (1) cognitive training, (2) physical training, (3) education program, (4) simulator training, (5) On-road driver training. Speed-of-processing and reasoning training resulted in lower rates of at-fault collision involvement than controls during cognitive training. Only the education program theme did not show a significant improvement, and the effectiveness increased when carried out in conjunction with on-road driver training. Driving performance was improved through physical training, simulator training, and on-road driver training. Conclusion : The articles reviewed programs for various types of interventions. It is important for occupational therapy practitioners to provide an appropriate interventions for elderly drivers to improve their driving performance. Therefore, a variety of Korean training programs for driving rehabilitation are expected to be developed.
