본 연구의 목적은 비만 성인 여성의 체중조절을 위한 컴퓨터 기반 인지행동치료 프로그램을 개발, 실시하여 그 효과를검증하는 것이다. 연구 대상은 BMI 23 이상에 해당하고 기타 임상적인 증상을 보이지 않는 성인 여성을 실험집단, 비
교집단(컴퓨터 기반 행동수정 프로그램 실시) 및 통제집단에 각각 8명씩 임의 배정하였다. 프로그램은 주 2회 총 16회
기이며, 개별적으로 웹사이트에 접속하여 과제를 수행하도록 되어 있다. 본 연구의 주요 결과는 다음과 같다. 첫째, 프
로그램 실시 후 사후검사 시 비교집단의 BMI가 유의하게 감소한 반면, 프로그램 종료 3개월 및 1년 추후검사 시 실험
집단의 BMI가 유의하게 감소했다. 둘째, 프로그램 실시 후 실험집단의 기대목표체중이 유의하게 증가하였고, 비합리적
인 신념 및 부정적 자동적 사고가 유의하게 감소했다. 건강자기효능감 중 심리적 효능감은 실험집단만이 유의한 증가
를 보였으며, 이는 프로그램 종료 3개월 및 1년 후에도 유지되었다. 셋째, 프로그램 실시 후 실험집단의 우울 및 불안
은 유의하게 감소했으며, 이는 프로그램 종료 3개월 및 1년 후에도 유지되었다. 넷째, 프로그램 실시 후 실험집단의 전
반적인 문제 섭식행동은 유의하게 감소했으며, 이는 프로그램 종료 3개월 및 1년 후에도 유지되었다. 반면, 비교집단은
프로그램 실시 후에 외부적 섭식이 감소했으나, 프로그램 종료 3개월 및 1년 후 정서적 섭식과 절제에 의한 섭식이 오
히려 증가했다. 스트레스 대처전략의 경우, 프로그램 실시 후 실험집단의 문제중심적 대처는 유의하게 증가한 반면, 정
서완화적 대처와 소망적 사고 대처는 유의하게 감소했다. 프로그램 종료 1년 후 실험집단의 사회적 지지추구 대처 또
한 유의하게 증가했다. 프로그램 실시 후 비교집단은 문제중심적 대처가 유의하게 증가한 반면, 소망적 사고 대처가 유
의하게 감소했으나, 프로그램 종료 1년 후 소망적 사고 대처만이 유의하게 증가했다. 통제집단의 경우 프로그램 실시
후 모든 변인에서 유의한 변화를 보이지 않았으며, 건강증진행위는 오히려 유의하게 감소했다. 본 연구는 내담자의 특
성을 고려한 컴퓨터 기반 CBT 프로그램을 개발, 실시해 그 효과를 검증한 예비 연구로 기존 면대면 방식이 아닌 온라
인을 활용한 새로운 방식의 CBT 진행을 시도하였다는 데 의의가 있다.
영어 초록
The purpose of this study was to develop a computer-based cognitive behavioral therapy program for weight control in obesewomen and prove its efficacy. The subjects of this study were chosen from obese women whose BMI scores were above 23 and
who had no other clinical problem except obesity. The 8 person for each group were randomly assigned into an experimental
group, a comparison group(received a computer–based behavioral control program) and a control group. For the therapy,
participants connected to designated computer-based websites and completed assignments twice a week for 8 weeks. The main
findings of this study are as follows. First, BMI score for the comparison group significantly decreased at the posttest assessment
while the score for the experimental group decreased significantly at the 3-month and 1-year follow-up tests. Second, after
receiving the program, the experimental group was marked with significant increase in the expected target weight while the level
of irrational beliefs and negative automatic thoughts decreased. Self-efficacy of health increased for both experimental group and
comparison group after the program, while psychological efficacy increased only for the experiment group, and the effect was
maintained at the 3-month and 1-year follow-up tests. Third, after receiving the program, the experimental group showed
decrease in the level of depression and unrest, and the effect was maintained at the 3-month and 1-year follow-up tests. Fourth,
after receiving the program, the experimental group showed decrease in the level of general problematic feeding behavior, and
the effect was maintained at the 3-month and 1-year follow-up tests. For the comparison group, the level of external feeding
decreased after the program, but emotional feeding and restrained feeding were found to increase at the 3-month and 1-year
follow-up tests. In case of stress coping strategies, problem-focused coping strategy increased while relaxing-emotion strategy and
wishful thinking strategy decreased for the experimental group. Social support-seeking strategy of the experimental group was also
found to increase at the 1-year follow-up test. For the comparison group, problem-focused coping strategy increased while wishful
thinking strategy decreased after the program. However, at the 1-year follow-up test, wishful thinking strategy was found to
increase significantly. The control group, at the posttest, was not marked with significant changes in all of the variables except
for the level of health promotion which, actually, was marked with a significant decrease. This study is significant in the fact
that it attempted a new way of conducting CBT with an online-based method rather than a traditional face-to-face method by
developing and conducting a computer-based CBT program and verifying its effect as a preliminary study.
