본 사례에서는 어머니의 일관성 없는 양육 환경으로 인해 욕구 충족 및 관심을 얻지 못해 함묵을 욕구 만족을 위한 수단으로 선택한 아동의 증상을 완화시키기 위해, 미술활동이라는 간접적인 안전한 의사소통으로 자신을 표현하여 아동으로 하여금 자신감을 갖게 하고, 더 나아가서는 어머니와의 관계개선을 목표로 모-자 미술치료를 실시하였다.
미술치료는 총 8회기를 실시하였으며, 5회기까지는 아동만 치료하다가 6-8회기 동안은 아동과 아동의 양육에 가장 큰 영향력을 미치는 어머니를 개입하여 모-자 미술치료를 실시하였다.
그리고, 아동이 퇴원한 1주일 후 전화통화를 통한 상담을 했다.
1단계(1-2회기)에서는 아동과의 라포 형성과 미술치료에 대한 관심 유발단계로 안정된 정서와 자기 표현에 자유스러울 수 있도록 하여 아동이 점차 치료 상황과 치료사에게 익숙해졌다.
2단계(3-5회기)에서는 상징적인 미술활동을 통한 내면 표출하기와 자신감 회복을 목표로 인형만들기, 난화기법과 자유화, 그림검사인 SDG와 HTP를 통해 표현하게 하여 주저함 없이 자신감 있게 매체를 탐색하고 자신의 느낌이나 감정을 언어로 표현하기 시작하게 되었다.
3단계(6-8회기, 전화상담)에서는 아동과 어머니와의 의사소통과 관계 개선을 목표로 신체상 본뜨기와 서로의 장점 찾고 편지 및 선물 주고받기의 기법을 활용하여, 특히 어머니를 처음으로 개입시킨 6회기에서는 그림검사를 통하여 어머니의 자기 통찰 및 자각이 일어났으며 이것을 계기로 아동은 어머니를 이해하게 되고 어머니 또한 자신의 잘못을 반성하고 아동을 진심으로 받아들이고 이해하게 되어 의사소통이 시작되고 화해를 할 수 있게 되어 회기 중 서로가 협동하고 서로의 생각으로 존중하고 격려하는 모습으로 변화되었다.
따라서, 본 사례는 함묵증 아동에게 어머니를 개입시킨 모-자 미술치료가 아동의 자신감을 증진시키고 어머니와의 관계개선에 긍정적인 효과가 있음을 보여주었다.

영어 초록

This study was to make them express themselves with the indirect and safe communication through the art therapy and build self-confidence. Furthermore, this study was to execute the art therapy for the purpose of improving the relations with the mothers.
This study has executed the art therapy for total 8 sessions, and the children were cured until 5 sessions, and then this study was to execute the mother-son art therapy, intervening the mother who had the largest effect on the children and upbringing the children during 6-8 sessions. And, this research consulted with them through the telephone call one week later the children were discharged from the hospital.
In the first stage(1-2 sessions), it was a stage of inducing the interest in the formation of
rapport and art therapy with the children. The children became accustomed to the state of therapy and therapist step by step, making them being free to stable emotion and self-expression.
In the second stage(3-5 sessions), these stages were to make them express through doll
making dolls, squiggle technique like scribbling technique and free style painting, SDG and
HTP, a sort of picture tests for the purpose of inside expression and rebuilding their confidence through the symbolic art activities. After all, they began to search the media without hesitation and express their feelings and emotion by language.
In the third stage(6-8 sessions, consultation by telephone), this stage was to utilize the
techniques like making a pattern for the body image and look for the advantages each other
for the purpose of the communication with the children and mothers and the relation
improvement. In particular, in the 6 session that the mothers were intervened first, there
arose the mothers' self-insight and perception through the picture test. Taking this opportunity, the children came to understand their mothers, and the mothers came to reflect on their faults and sincerely receive and understand their children. Also, it started to communicate each other, and they came to terms with each other. After all, it has changed as aspects that they cooperated and respected individual thoughts each other during the session.
Accordingly, this study showed that the art therapy that the mothers were intervened had a positive effect on the children with selective mutism.
