서 론요낭종은 신장주변(perirenal) 또는 요관주변(paraureteral) 관외로 흘러나온 소변의 피포성 집합 (encapsulated collection)으로[1] 신장주변에서부터 후복막에 걸쳐서 발생할 수 있다[2]. 대부분 신장의 외상에 의해 발생하며 저절로 호전 보이는 경우가 대부분이며 극히 일부에서 요낭종로 진행한다.[3] 그외 요로결석, 방광요관역류, 방광폐쇄, 복부 종양, 임신 등으로 인한 요관폐쇄, 선천적 요인에 의한 역압에 의해 발생 할 수 있다[2,4,5,6].저자는 81세 남자 환자의 요로결석에 의한 급성 신손상을 동반한 요낭종(소변종) 사례를 보고하고자 한다. 본 사례에서 저자는 환자의 진단을 장요근 농양으로 판단했으며 치료적 목적으로 농양주위로 돼지 꼬리형 카테터를 삽입 후 배액성분 분석을 통해 소변임을 확인 후 장요근 농양이 아닌 거대 요낭종으로 진단하였다. 이에 다른 질환에 대한 감별 진단이 중요함과 국내에서 급성 신손상을 동반한 거대 요낭종이 보고되지 않아 문헌고찰과 함께 보고하고자 한다.

검사 결과상 각각 배액물 성분이 동일, 즉 소변임을 확인 할 수 있었으며 배양검사상 동정되는 균은 없었다. 3일 동안 돼지 꼬리형 카테터 배액 후(1일 3000ml, 2일 1000ml, 3일 200ml미만) 점점 배액양 감소(100cc미만) 하였으며 추적 혈액검사상 크레아티닌 수치 감소(3.1mg/dl-> 1.1mg/dl)로 급성 신손상 호전보여 요낭종 추적 검사위해 조영증강 컴퓨터 단층촬영 시행하였다(Fig. 3 )
좌측 장요근을 따라서 관찰되던 다발성 중격의 크기와 양이 줄어든 상태였으며 매일 배액되는 양 감소 및 추적 혈청 크레아티닌(Cr) 1.1 mg/dl 이하로 지속적으로 감소하여 급성 신손상 원인은 좌측 하부요관에 있는 0.6cm 요관석으로 인한 역압에 의한 수뇨관 신증으로 인한 신후성 급성 신손상(Post renal AKI)임을 알 수 있었다.
