➀ Definition
주요 우울증(major depressive disorder, MDD)은 임상에서 접하게 되는 가장 일반적인 정신과적 질환 중의 하나로서 지속적인 불쾌감, 불안감, 과민감, 두려움, 식이 및 수면장애, 체중이상, 정신운동의 동요(agitation) 또는 둔화(retardation), 활력저하, 무가치나 죄의식을 느끼며, 집중 또는 사고의 곤란, 섬망, 환각, 자살충동 등으로 특징지어지는 질환으로서 전연령층에서 발생되며 수일에서 수개월에 걸쳐 증상이 지속될 수 있고 증상은 연속적으로 발생하거나 몇 년간의 정상생활 중 간헐적으로 나타날 수 있다. 주 우울증은 단극성 장애(unipolar diorder) 또는 정동성 정신장애(major affective disorder)로도 불린다.
주요 우울장애는 조증 삽화, 혼재성 삽화, 경조증 삽화의 병력 없이 단 한 번이나 그 이상의 주요 우울증 삽화를 보이는 경우를 말한다.

1) Diagnosis of DSM-IV criteria
주 우울증의 진단은 미국 정신질환협회의 Diagnostic and Statistical Manual of Mental Disorders, 4th Edition (DSM-IV) criteria에 따라 (A)우울감이나 (B)무쾌감증 (anhedonia) 중의 1가지를 포함하여 다음 (1)번부터 (7)번까지의 증상 중 5가지 이상이 2주 이상 지속될 때 진단된다.
