재난적 의료비 신규 발생 가구의 결정요인 분석

본 연구는 재난적 의료비의 신규 발생을 결정하는 요인을 파악하는데 주요한 목적이 있다. 이를 확인하기 위하여 2015년도와 2016년도 한국의료패널조사 자료(2016년은 베타버전)를 분석하였다. 연도별로 역치 10% 수준에서 재난적 의료비 가구의 발생률을 확인하고, 2015년과 2016년도 자료를 통합하여 재난적 의료비가 발생한 가구의 결정요인을 파악하였다(연구모형1). 최종적으로 재난적 의료비 상태에 새롭게 처하는 가구가 어떤 특성이 있는지 확인하였다 (연구모형 2).재난적 의료비 가구의 발생률(역치 10%)은 2015년 37.5%, 2016년 38.5%로 한 해 동안 1.0%p 증가하였다. 두 개년도 자료를 통합한 분석에 따르면, 주요한 가구주의 특성과 가구의 특성이 가구가 재난적 의료비 상태에 처하는 데 영향을 미쳤다. 한편, 가구주와 가구의 여러 가지 특성 중에서 가구주의 성별, 교육수준, 취업상태, 배우자 유무, 주관적 건강상태, 가구원 수, 가구 내 만성질환 개수, 가구 내 65세 이상 노인 유무는 재난적 의료비 가구의 신규 발생에 영향을 미쳤다.

영어 초록

This study analyzes the determinants for households experiencing catastrophic health expenditures. To confirm the determinants, we analyzed the Korean Health Panel Survey data for the years 2015 and 2016(beta version). Incidence rates for catastrophic health expenditures were estimated at a 10% threshold for each year, and the determinants of households experiencing catastrophic health expenditures were identified by integrating data for 2015 and 2016 (Study Model 1). Finally, we examined the characteristics of households newly exposed to catastrophic health expenditures (Study Model 2).The incidences of catastrophic health expenditures (based on a 10% threshold) was 37.5% in 2015 and 38.5% in 2016, an increase of 1.0%p over the year. According to an analysis of two years of data integration, the characteristics of major household heads and household characteristics influenced catastrophic health expenditures. However, sex, educational standard, state of employment, marital status and subjective health status of household heads, the number of household member, number of chronic diseases in households, and the elderly (people aged 65 years or older) living in households affected new incidences catastrophic health expenditures.
