Objective: This study was aimed to compare maternal and neonatal clinical parameters between with and without histologic chorioamnionitis in women diagnosed with preterm premature rupture of membranes(PPROM).Methods: This retrospective case-control study included women with PPROM from 23+0 to 36+6 weeks of gestation, who delivered at Korea University Hospital in Ansan from Jan. 2006 to Dec 2017. The maternal characteristics, neonatal characteristics and neonatal outcome variables were compared between histologically confirmed chorioamnionitis and control group. Diagnosis of histologic chorioamnionitis was made using histologic examination of placentas.Results: A total of 166 women with PPROM were analyzed. There were no significant differences between two groups in latency, oligohydramnios, diagnosed IIOC, positive vaginal culture and cervical length. However there were significant differences in CRP, WBC and neutrophil% between two groups. Clinical chorioamnionitis was highly correlated with histologic chorioamnionitis (r=0.385, p-value<0.001). Incidence of placental abruption was significantly lower in histologic chorioamnionitis (p=0.012). Neonatal morbidities (5min Apgar score less than 7, ventilator care, sepsis, NEC, RDS, PVL, BPD, IVH grade 3 to 4) demonstrated no significant difference between two groups. The rate of neonatal death was significantly higher in histologic chorioamnionitis group(p=0.04). Logistic regression analysis revealed no significant relationship between chorioamnionitis and neonatal death.Conclusion: Histological chorioamnionitis is associated with higher WBC, neutrophil count and CRP at admission in women with PPROM, but no significant increased adverse neonatal outcomes were observed. Women with PPROM and chorioamnionitis are less likely to be associated with placental abruption compared to PPROM without chorioamnionitis.
