Linderman(1994)에 의해 처음 언급된 preparatory grief는 죽음 또는 임종의 단계를 시작하게 만드는 분리의 과정에서의 위협을 말한다. 구체적으로 이야기 하자면 reactive grief는 과거 상실에 대한 비탄을 이야기 하는 반면, preparatory grief는 손자의 출생을 보지 못하는 것과 같은 미래의 상실에 대한 비탄을 말한다.(Rando 2000., Periyakoil&Hallenbeck 2002.) 이는 임종의 단계에서 겪게 되는 정상적인 과정이며 죽음이 가까워 올수록 그 강도가 강해지게 된다. Preparatory grief와 더불어 우울한 감정과 절망, 불안은 말기암 환자들에게서 많이 볼 수 있는 증상이다.
현재는 말기암 환자의 preparatory grief에 있어 우울, 절망 그리고 불안 간의 상호관계에 대해 밝혀진 것이 거의 없다. 따라서 이 연구는 preparatory grief에서 우울, 절망, 불안이 어떻게 영향을 미치는지 뿐만 아니라 그들 상호간의 관계에 대해 밝혀내기 위해 시행되었다.
Patients
Athens의 완화의료센터를 다니는 외래환자를 대상으로 다음의 기준을 만족시키는 사람을 연구대상자로 정하였다.: 병리학적으로 악성종양으로 판정이 났으며 18세 이상, 조사자와 효과적인 의사소통이 가능하며 질병진단에 관한 지식이 있는자를 대상으로 동의서를 받아 시행하였다. 단, 심리학적인 문제가 있거나 심각한 인지장애가 있는 경우 제외되었다.
